New Patients

Welcome! We're honored that you've chosen Dr. Antonio Monzon for your healthcare needs. To help us provide you with the best possible care, please review the following information.

What to Bring to Your First Visit

  • Photo identification (driver's license, state ID, or passport)
  • Insurance card(s) - front and back
  • Completed patient forms (see downloads below)
  • List of current medications - including dosages
  • List of allergies - medications, foods, latex, etc.
  • Previous medical records - if available and relevant
  • Referral form - if required by your insurance
  • Payment - co-pay or deductible due at time of service

Arrival Time

Please arrive 15-20 minutes before your scheduled appointment time. This allows time to complete any remaining paperwork and update your information. If you're running late, please call our office so we can advise if rescheduling is needed.

What to Expect at Your Visit

Annual Well-Woman Exam

Your annual exam is a comprehensive check of your overall health. It typically includes:

  • Review of your medical history and any concerns
  • Blood pressure, weight, and vital signs check
  • Breast examination
  • Pelvic examination (when appropriate)
  • Pap smear and HPV testing (based on guidelines)
  • Discussion of contraception, if desired
  • Lab work orders as needed

Prenatal Visits

Prenatal care is essential for a healthy pregnancy. Visit frequency:

  • Weeks 4-28: Every 4 weeks
  • Weeks 28-36: Every 2 weeks
  • Weeks 36-delivery: Weekly

High-risk pregnancies may require more frequent visits.

Insurance & Billing

We accept most major insurance plans including:

Payment Information

  • Co-pays and deductibles are due at time of service
  • We accept cash, personal checks, and major credit cards (Visa, MasterCard, Discover, American Express)
  • Payment plans may be available for larger balances
  • Self-pay patients: Please contact our office for pricing

Office Policies

Appointment Policy

We understand that life happens. If you need to cancel or reschedule, please provide at least 24 hours notice. This allows us to offer your appointment time to another patient who needs care. Repeated no-shows may result in dismissal from the practice.

Late Arrivals

If you arrive more than 15 minutes late for your appointment, you may need to be rescheduled to ensure all patients receive adequate time with the doctor.

Prescription Refills

Please allow 48-72 hours for prescription refill requests. Contact your pharmacy first, and they will send the request to our office. Some medications require an office visit before refilling.

Medical Records

To request copies of your medical records, please complete a Medical Records Release form. Allow 7-10 business days for processing. There may be a fee for copying and handling.
